Please go to our Links Page to download the most recent version. STEP 2: Please read the FCRA Summary of Your Rights *Note - The Summary of Your Rights is located on our Links Page. STEP 3: Fax the Required Forms to (703) 340-1695 The required forms include: 1) Online Fax Cover Sheet 2) Online Authorization Release Form 3) FCRA Notice and Disclosure Form 4) A clear copy of your Driver's License or Government Issued Identification (front and back of card). NOTE: UPON COMPLETION OF THE BACKGROUND SCREENING, A COPY OF THE REPORT WILL BE SENT TO YOU BY EMAIL IN PDF FORM. THE SCREENING WILL ALSO BE SENT TO THE COMPANY YOU ARE APPLYING FOR IF YOU HAVE AUTHORIZED THIS ACTION. |
